middletownMiddletown Park Rehabilitation and Health Care Center, formerly known as Park Manor, was founded in 1971.  After five years of planning and obtaining required approvals, construction was started in September 1976.  In May 1978 the first 130-beds were completed and the doors were opened to the community.

In 1995, a one hundred and twenty bed, state of the art addition was built onto the existing facility.  The facility later changed it’s name to Park Manor Rehabilitation and Health Care Center in 2002, to reflect the changing nature of services provided to residents and the community.  For more than 30 years, Park Manor has continued it’s history of excellence  and personalized care as one of the largest and most progressive health care providers in Orange County.

In March 2010, Park Manor Rehabilitation and Health Care Center underwent a change in ownership and is now operating and known as Park Manor Acquisition II, LLC, (dba, Middletown Park Rehabilitation and Health Care Center).  As part of the Epic Health Care Management family, we have the shared vision to protect, preserve, and improve the physical, psychological and  environmental health of all owned and operated facilities, through professional, cost-effective Health Management Services, while valuing patient’s dignity and quality of life.
